The One Question Physicians Are Terrible At Answering

What area of your life could use more attention right now?
Go ahead. Answer it. No overthinking.
If you just went blank, or if "everything" was the first thing that came to mind before your brain kind of gave up and moved on, you are not alone. And you're not broken. You're just someone who has spent years, maybe decades, being trained to push through, solve problems fast, and keep moving. The idea of pausing to honestly assess what you personally need is not exactly built into the curriculum.
But here's the thing: that pause is exactly where things start to change.
I spend a lot of time working with physicians, as a physician myself and as a coach. And across that work, I've noticed that most of us are struggling in one of five areas. Not five randomly chosen categories, but five specific places where the combination of our training, our conditioning, and the broken systems we work inside tends to create the most friction.
Health. We know more about the human body than almost anyone, and we are often the worst at taking care of our own. We've been trained to ignore hunger, skip sleep, push through pain, and deprioritize our own medical care. If your health has been the thing you keep meaning to get to, that's worth naming.
Boundaries. What's okay and what's not okay, a deceptively simple framing that gets incredibly complicated in our context. We work in hierarchies. We've been socialized, many of us, to accommodate. We've faced real pushback when we've tried to draw lines. If boundaries feel both necessary and dangerous to you, that's not a character flaw. That's a very rational response to real experiences.
Emotional well-being. This one has a wide range. Maybe you're more irritable than you want to be and you'd like to work on that. Maybe you're dealing with something that genuinely needs professional support. Maybe your relationship needs attention. Wherever you are on that spectrum, the fact that we've been conditioned to shove our emotions down and keep performing doesn't mean those emotions stopped mattering.
Time. Not just "I'm so busy" as an abstract complaint, but actually getting honest about where your time is going. Both the time you feel like you control and the time you feel like you don't. Spoiler: we often have more agency over both than we think we do.
Career arc. There are seasons where your career needs focused attention. Where you're planning a move, building something new, figuring out what comes next. If that's where you are right now, it deserves real time and thought, not just the mental background noise of "I should probably figure this out eventually."
Here's my ask: don't try to work on all five. That is our default, and it doesn't work. Instead, read back through that list and notice which one produces the strongest reaction in you. Not the one you think you should pick. The one your gut already knows.
Sit down with it. Write down where you currently are. Write down where you want to be. Write down one or two things you could try in the next four to six weeks. That's not a small thing. That's actually how change happens.
