The patients loved you. You were the one who stayed, who took one more, who never let anything drop. The system was never satisfied, and you kept going anyway, because that is what you were trained to do.

Head down. Unboundaried.

It worked, and it cost you.

Nobody ever taught you to trust your own read on your own practice.

So now there's something you keep circling. Maybe you've said it out loud as a maybe. Maybe you haven't said it at all, because you can hear how it would sound, and you already know who would have opinions about it.

And when you do go looking for help, what you find is a checklist for opening a practice. Useful, eventually. Not the thing that's actually stopping you.

You want to give better care than you're able to give right now. You also need the math to work. You've probably noticed that almost nobody talks about both of those at the same time.

What I actually do

In medicine we send patients to each other. When my patient needs something I'm not trained in, I want to know where to send her, so I don't have to say no to something I know would help.

There's no referral pathway for a physician who's stuck. Or leaving. Or opening something and terrified of it.

That's what I do.

You probably already have somewhere to go for the tactics. A mastermind, a course, a consultant, a friend who's done it before. Good. Keep them.

What I hold is the human piece in the middle. What you actually value. What you want your days to feel like, on a Tuesday, not in a five year plan.

We'll talk about money, and pace, and the thing you keep not doing. The rate you won't raise. The contract you never went back to. Same problem, different setting.

What your care is worth and how you spend a Tuesday are the same conversation as what you value. Most people will only ever help you with the first half of that.

What actually happens on the call.

You book. You’ll get one question ahead of time, about what’s bringing you here. Answer it badly if that’s what’s honest. “I don’t know, something’s off” is a real answer, and I’ve heard it plenty.

Then we talk. 20 minutes, just the two of us.

You might ramble. You might spend the first ten minutes explaining why it isn’t the right time, or why other people have it worse. That’s normal. It’s usually where the real thing is hiding.

My job is to listen for what’s rattling around in there, and to help you say it out loud. Often for the first time.

Nothing leaves the call. You don’t need a plan, and you don’t need to have decided anything. You just need the thing you keep not saying.

Who I am

I’m Dr. Megan Melo. Family and Obesity Medicine, certified by The Menopause Society. Mom to a teenager and a tween. Wife. Recovering perfectionist.

I spent years trying to fix myself with self-help books, passive courses and therapy. Therapy I would recommend to any physician. All of it helped. None of it had me setting a boundary and keeping it, or making a career decision that actually fit me. Coaching did that.

I left employed practice and built my practice and my brand inside someone else’s small private practice, as a contractor. Then the coaching business, and a podcast, to support physicians the way I had wanted to be supported. I know what it is to build something of your own without burning down what you already have.

A lot of my patients are physicians. They come to me for their own care.
